Program of All-Inclusive Care for the Elderly – Jordan Valley Health Center

The Jordan Valley Health Center will soon launch a new program called Program of All-Inclusive Care for the Elderly, also known as PACE.

The Program of All-Inclusive Care for the Elderly will help participants receive the health care needed, as well as solves challenges for eligible seniors.

The Program of All-Inclusive Care for the Elderly provides PACE participants with a team of providers and health navigators who create a custom care plan to help the particiant stay healthy. The Program of All-Inclusive Care for the Elderly services include Primary Medical Care, Medical Specialists, Home Care, Physical, Occupational and Speech Therapy, Dental Services, Vision Services, Nutrition/Diet Guidance, Nursing Care, Wheelchair Accessible Transportation to and from all care-related activities., Personal Care, Pharmacy/Medications, Labs and X-Ray, Memory Care, Dialysis, Emergency and Hospital Care, 24-hour Care Support, and potentially more needed services through approved providers.

The Jordan Valley Senior Care PACE, also known as JVSC Day Center, is located at 1720 W Grand St. in Springfield, Missouri.

According to the Missouri Department of Social Services, “A PACE Organization is responsible for providing care that meets the needs of the participants in all aspects of life, 24 hours a day, every day of the year. All PACE centers include a primary care clinic and areas for participants to receive needed therapies, socialization, personal care, and dining services. With the help of an interdisciplinary team, the PACE center also serves as the focal point for the coordination and administration of most PACE services.”

Jordan Valley Senior Care is the first PACE program to open in southwest Missouri.

The PACE program is typically funded by Medicare and Medicaid to deliver necessary care. All your necessary healthcare services may be provided if you are covered by Medicare and/or Medicaid.

To be eligiable to patiricpate in the PACE program, a potential particiant must be 55+ years of age, certified to need nursing home care, live in the Jordan Valley Senior Care services area, and be able to live safely in the community with JVSC services at the time of enrollment.

According to a news release from KY3, the program is launching the assessment and enrollment process and should begin caring for participants in July.
